NURSEFINDERS REFER A COLLEAGUE PROMOTION
NOVEMBER 1, 2007 - AUGUST 31, 2008

  1. NO PURCHASE NECESSARY TO ENTER OR WIN. CHANCES OF WINNING DEPEND ON NUMBER OF ELIGIBLE REFERRAL ENTRIES RECEIVED. THIS PROMOTION IS SPONSORED BY NURSEFINDERS, INC., 524 E. LAMAR BLVD., STE. 300, ARLINGTON, TX 76011 (“NURSEFINDERS”). OPEN TO EMPLOYEES OF NURSEFINDERS, NATIONAL HEALTHCARE STAFFING, TVL HEALTHCARE, LINDE HEALTHCARE, KENDALL & DAVIS, AND CLUBSTAFFING (EACH A “NURSEFINDERS COMPANY”) WHO ARE CAREGIVERS GENERATING BILLABLE HOURS FOR A NURSEFINDERS COMPANY AND WHO RESIDE AND WORK IN THE U.S. (“NURSEFINDERS U.S. CAREGIVERS”). NOT OPEN TO ANY EMPLOYEES OF ANY NURSEFINDERS COMPANY EXCEPT FOR NURSEFINDERS U.S. CAREGIVERS. ALSO OPEN TO U.S. RESIDENTS OVER THE APPLICABLE AGE OF MAJORITY WHO ARE NOT EMPLOYEES OF ANY NURSEFINDERS COMPANY. VOID WHERE PROHIBITED OR RESTRICTED BY LAW. PROMOTION BEGINS NOVEMBER 1, 2007 AND ENDS AUGUST 31, 2008 (THE “PROMOTION PERIOD”).
  2. ELIGIBILITY: Promotion open to employees of a Nursefinders Company who are Nursefinders U.S. Caregivers. Promotion not open to any employees of any Nursefinders Company except for Nursefinders U.S. Caregivers. Promotion also open to U.S. residents who are not employees of any Nursefinders Company and who are over the applicable age of majority at the time of referral and entry.
  3. TO ENTER: Eligible referrers can enter during the Promotion Period by referring, as a potential new Nursefinders employee, a nurse, allied health professional, and/or physician who has not worked for a Nursefinders Company in the 6 months prior to the referral. To refer potential new employees, accurately complete and submit an electronic referral using the “Refer a Colleague” form at the www.yourgreennetwork.com website or make a phone referral by calling 1-866-691-5678. Each qualified referral will get the referrer 1 entry in the drawing, provided that the new employee works for a Nursefinders Company for at least 80 hours during the Promotion Period. For each additional 80 hour block that the new employee works for a Nursefinders Company during the Promotion Period, up to a total of 13 additional weeks, the referrer will get 1 additional entry, for a total of up to 6 entries based on that initial referral. If the new employee becomes a permanent placement for a Nursefinders Company (that is, the new employee becomes a permanent employee of a healthcare provider with a Nursefinders Company as the placement agency) during the Promotion Period, the referrer will get 2 additional entries, for a total of 3 entries based on that initial referral.
  4. SELECTION AND NOTIFICATION OF WINNER: The winner of the prize will be selected in a random drawing of all eligible entries conducted on or about September 15, 2008. Chances of winning depend on the number of eligible entries received. The winner will be notified by phone and/or mail, on or about September 30, 2008. If the winner cannot be located, or is deceased, the prize may be forfeited and an alternate winner selected. Nursefinders reserves the right to validate information provided on any referrals and entries before awarding the prize. All participants agree to the use of their names, likenesses, and photographs, within and outside of any Nursefinders Company, in connection with this promotion.
  5. PRIZE: The winner of the drawing may choose either a Toyota® Prius® (Approximate Retail Value - $25,000) or a payment of $25,000. The winner is solely responsible for any taxes, costs, or other expenses associated with any prize, except that, if the winner chooses the Toyota® Prius®, Nursefinders will pay the sales tax, the initial tag fee, and the initial title fee. If a prize cannot be awarded for any reason, Nursefinders reserves the right to substitute another prize of similar value. The prize is not transferable.
  6. GENERAL CONDITIONS: All referrals and entries become the sole property of Nursefinders and will not be acknowledged or returned. All referrals and entries are subject to verification. Nursefinders and each Nursefinders Company assume no responsibility for referrals or entries that are lost, late, misdirected, stolen, illegible, undelivered, garbled, damaged, mutilated, incomplete, forged, falsified, altered, or tampered with (and any such referrals and entries are ineligible), or for any computer, online, telephone, or technical malfunctions that may occur. Nursefinders and each Nursefinders Company assume no responsibility for any technical problems in connection with the transmission or receipt of referrals or entries, including faulty electronic data transmission, traffic congestion on the Internet or on websites, hardware or software error, equipment failure or malfunction, or other online communications problems. In the event of a dispute regarding the identity of the referrer submitting a referral or entry, the referral or entry will be considered made by the person who identifies himself or herself on the referral form or in the referral phone call. The winner may receive an IRS Form 1099 or other tax form for the value of the prize. Nursefinders and each Nursefinders Company assume no responsibility or liability for the receipt or use of any prize. Nursefinders and each Nursefinders Company reserve the right to publish the names, photographs, and likenesses of all entrants and the winner. If for any reason this promotion is not capable of running or being completed as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other cause which corrupts or affects the administration, security, fairness, integrity, or proper conduct of the promotion, Nursefinders and each Nursefinders Company reserve the right, at their sole discretion, to disqualify any participant who is responsible or who tampers with the promotion, and to modify, cancel, or terminate the promotion and select the winner from all eligible entries received prior to the cancellation or termination. CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION MAY VIOLATE CRIMINAL AND CIVIL LAWS. This promotion is subject to all applicable laws and regulations. Void wherever prohibited or restricted by law. Entrants agree to abide by the promotion rules and the decisions of Nursefinders and each Nursefinders Company, which are final.
  7. WINNER: The winner’s name (and, at Nursefinders’ discretion, the winner’s likeness and photograph) will be posted at www.yourgreennetwork.com and at the website of each Nursefinders Company.
